Registration Process:
All children, regardless of race, color, religion, sex or national origin, are welcome at The Gathering Place. The following information is needed to enroll your child:

1. Completed registration forms
2. The prescribed Child Enrollment and Health Information form must be completed for the first day of attendance (State PDF form)
3. Immunizations record on file in the Center's office (State PDF form)
4. Permission to transport to a medical center
5. Physician signed and dated medical form (must be submitted within 30 days of enrollment) (State PDF form)
6. Completed EFT form or credit card information
7. Paid, non-refundable registration fee
8. Paid, refundable material fee
9. First week's tuition (non-refundable)
10. Income eligibility applicaton
11. Copy of child's birth certificate, custody papers, etc., as needed
